MESSAGE
FROM THE SECRETARY

The recent news from Kerala is very encouraging. The Kerala Government has issued an order incorporating palliative care into the primary health care system in the state. You all know that the Kerala is the only state in India having a declared palliative care policy and this new order is a logical squeal of the policy.

This new order gives clear guidelines on setting up palliative care services in the primary health care sector, training and reporting. Palliative care is designated as a regular activity of the primary health care system in the state. Making palliative care as part of the primary health care may be a more logistic option to make the care available to many millions needi-

ng it.  This may true not only about Kerala or India but many countries in the world.

These government actions did not happened over night. It took many years of hard work and persistent follow up from the palliative care community in the state to reach this welcoming position. The government mechanisms are also showed much support to the programmed. The palliative care community thankfully recognizes the support extended by the Health Minister of Kerala Mrs. P. K. Sreemathi Teacher and the previous Secretary of Health Mr. Viswas Mehta along with other government officials to the cause. The second phase of the palliative care programmed for the state under the National

Rural Health Mission can ensure the adequate implementation of this order.

More on this new path breaking government order can be read elsewhere in this newsletter and I hope that the other state governments in India also will make a note of these developments.

The Central Council of IAPC will be meeting in Bangalore later this month. For the last few years the CC is able to meet mid annually. This has helped us to plan and review our activities in detail as we will have a whole day for the meeting.

BANGALORE HOSPICE TRUST - KARUNASHRAYA

A View of Karunashraya
 
   

KARUNASHRAYA, meaning an abode of compassion, is a hospice set up in March 1995.  Its aim is to care for advanced stage cancer patients, who have been pronounced to be in the last stages of life. When a patient comes in for symptom control, pain relief or for other sort of palliative care, close relationship has been developed between the hospice staff and the patient and his family and it continues with Home Care. During the last days of the patient, when he/she  is readmitted to the hospice, this love and care from the hospice staff can be seen in in-patient care and this ends only after a bereavement visit after the patient has passed away.

The 50 bed hospice in Bangalore spread over a five-acre plot, replete with gardens, lawns and the wards overlooking a water body.  Here palliative care is given not only to the patients, but their families also.  The service includes physical, psychological, social and spiritual care which is provided by an inter-disciplinary team of dedicated staff that includes Doctors, Nurses, and Nursing Health Assistants, counsellors, social workers, office staff and volunteers.

The goals of the institution are:

  1. To alleviate pain and related distressing symptoms.
  2. To ensure quality of life at every step and to counsel the patient and family in order to give them courage to face the inevitable.
  3. To heighten awareness of palliative care.
  4. To enable other similar institutions to render palliative care by training and encouraging their staff
    In the last 10 years Karunashraya cared for over 8,600 advanced stage cancer patients and out of them 5,800 have died, peacefully and with dignity.
 

HOME CARE

The home care   service is being provided in and around Bangalore.  The Home Care teams comprise of nurses and social workers who help patients to receive quality care at home.  Here too psychological support plays an important role.  Once patients find it difficult to manage at home they are referred to the in-patient service of the hospice. 

TRAINING  PROGRAMME

Karunashraya also render yeomen service to the community by training young girls, mainly from rural/less privileged backgrounds, and make them into health assistants. They are eventually useful to any sort of patients at their respective residences thereby providing the youngster a specialized vocational skill and the ailing patient the much needed and desired professional help.

SATELLITE PALLIATIVE CARE CENTRES

Seven other Palliative Care centres have been developed by various other organizations under the training and guidance of Karunashraya.  These are at Chennai, Vellore, Shimoga, Kallianpur, Puttur, Nagpur and Mangalore.

Karunashraya is the local coordinator for the teaching programmes of the Cardiff University palliative care Diploma and M Sc.

10th ANNIVERSARY CELEBRATION

   
Karunashraya Celebrated its 10th anniversary on 1st May ,2009. Mr. Ramesh Aravind , eminent cine artist was the Guest of Hounour. Patients ,their relatives and the relatives of patients who received care from this hospice in their last days also participated in the celebration held at the premises of Karunashraya. Patients and their relatives shared their experiences.
Speical awards were given to staff for long service and special contributions were recognised.
The evening ended with a cultural programme by Vikram Suri & Nimitha Rao from Surya Kalavidaru .

Kerala Government incorporates Palliative Care into
Primary Health Care

 

Palliative Care to Purulia & Adra

   

The Director of Health Services, Government of Kerala issued an order on 29th of July to incorporate palliative care into the primary health care system in the state. This logical step is a follow up to the Government’s palliative care policy.

The Government order spells out the ‘standard operating procedure’ including responsibilities and the system of reporting. Incorporation of palliative care training in the in-service training programs, establishing nurse led home care programs in palliative care as part of regular activities of the primary health care system, initiating steps to establish inpatients beds for palliative care in community hospitals and district hospitals. District medical Officers in all the districts have been given the responsibility of making sure that doctors, nurses and the field team are adequately trained. Medical officers in charge of Primary Health Centrs and Community Health Centres have been instructed to make arrangements for conducting outpatient clinics once a week exclusively for patients in need of palliative care and to make sure that medicines needed for palliative care are available in the health center/hospital.

The new initiative by the Government will also facilitate National Rural Health Missions’s  (kerala) attempts to initiate palliative care programs in all villages and to integrate the service delivery from various agencies at the grass root level.

With this government order , Government of Kerala has probably become the first Government in the whole world to formally incorporate palliative care in to primary health care. This is considered as a very encouraging step for public health activists in palliative care all over the world.

 

 

‘KOSISH’- the only Palliative Care Service in Jharkhand has started providing services in the remote areas in the nearby state of West Bengal. As part of this community based geriatric services has been started in the districts of Purulia and Adra of West Bengal, which are considered to be the most backward districts in the state. The service being provided is totally free of cost.

At the beginning of the programme awareness camps were conducted at Purulia and the Railway colony at Adra. ‘Koshish’ targeted remotely located villages in Purulia district, at the foothills of the Ayodhya Mountains, with no motorable roads where most of the people are below poverty line. The tribals in this region are mostly Santhals.

The Palliative Care team   found a large number of the elderly afflicted with uncontrolled hypertension (undiagnosed) & cerebral strokes. There were people and families suffering from other non-malignant conditions like COPD, nutritional deficiencies, arthritis, cerebral palsy, convulsive disorders etc.

Eventhough reaching to the villages in the middle of the forest is very difficult; ‘Koshish’ provides service to this community once in every 15-20 days.   Free medicines are being distributed in each visit.

 One-month Certificate Course for  Doctors Nurses and Volunteers


MNJ Institute of Oncology and RCC completes third successful year of running the One-month Certificate Course for Doctors, Nurses and Volunteers. Regional Workshop at Bangalore
 

It was decided early this year in Delhi to prepare a question bank to benefit the candidates appearing for the IAPC Certificate Course and   also to aid the examination process. In this regard a workshop for the national faculty members form south was held by Indian Association of Palliative Care at Karunashraya , Bangalore on 14th June 2009.
Nineteen National Faculty Members from South Zone participated in the workshop. Mr.  Kishore S. Rao, Managing Trustee of the Bangalore Hospice Trust- Karunashraya, inaugurated the workshop. Dr. Rajashree, Chairperson, Board of Examination, IAPC Certificate Course, explained programme for the day and gave the necessary guidelines to prepare the question bank.
The faulty members worked in groups to prepare questions under different topics. The question bank covering the entire reading materials for the course will be available from 1st week of July at the website of IAPC, www.palliativecare.in A soft copy of this will be sent to all regional coordinators and print outs can be given to candidates if needed.

Palliative Care Insurance for Patients with Cancer below Poverty Line


The government of Andhra Pradesh has recently started a health insurance system for people with cancer below poverty line, a first of its kind in the country. The big achievement is that palliative care is also included in this scheme. Any patient with cancer who needs palliative care and belongs to poor socio-economic status gets entire treatment free.

 Palliative Care for Children with Cancer: An  initiative by MNJIO and RCC


A comprehensive paediatric oncology program in MNJ Institute of Oncology and RCC has seen a rapid expansion since its inception last year. On an average, 40 new children are seen every month from the different districts of Andrapradesh and neighbouring states. A dedicated palliative care team consisting of doctors, nurses and social workers offers active palliative care to children and their family. Some of the highlights of the program are the following:

 
 
  • Enrolment of every child into palliative care from the day of diagnosis
  • Supportive care during curative treatment
  • Procedural pain management and acute pain management policy
  • End of life care
  • Weekly support group meeting for the parents, siblings and sick children
  • Recreational activities for children
  • Free treatment throughout the course of treatment and free food for children and their family during the stay in the hospital

 IAPC Certificate Course- An Update

 
didates registered is more than a thousand. Now the fourth batch has started on 1st June and this time this time we have 380 candidates registered through 26 centres across India and both these numbers are increasing.
From the feed back received from regional centres, reasons suggested for the popularity of this course were
  • Mode of the course- Distance Education: This helps professionals to attend the course and study the basic principals of the       course without interrupting their regular practice/service
  • Quality of Course Material: It is concise but comprehensively covering the basics of palliative care
  • Quality of lectures delivered by the Members of the National Faculty Pool created by the Indian Association of Palliative Care

Delegates at the launch of the Worldwide Palliative Care Alliance (WPCA)

 

     

The Worldwide Palliative Care Alliance (WPCA), a new global action network designed to focus exclusively on hospice and palliative care development worldwide was formally launched on 5th May 2009 at the 4th Worldwide Summit of National Associations of Hospice and Palliative Care in Vienna, Austria.

The WPCA is an alliance of national and regional hospice and palliative care organisations, bringing together people around the world with a shared vision of universal access to quality palliative care. WPCA will provide a global voice to achieve this goal and undertake various activities including advocacy, education and training, policy developm-

 
ent and research.

National hospice and palliative care organisations have been informally meeting every two years since 2003 to address this need – this group has now formally become the WPCA. The formalization of this network is an important step forward in making sure all those who need quality palliative care can access it.

World Hopice And Palliative Care Day

     

This year world hospice and palliative care day will be observed on October 10th.

The theme for  World Day 2009 is “Discovering your Voice”
This Day is for people to engage in events and activities to raise awareness and funds to support the development of hospice and palliative care. All around the world, there are people who cannot access it.
This year, the voices of people living with life-limiting illness, their carers and families will be heard to show the importance of hospice and palliative care and what it means to them.
Policy makers and key people who can influence the development of hospice and palliative care will be encouraged to speak out about this important issue.

 
 

The major fundraising activity for World Day 2009 is Voices for Hospices , a wave of concerts and musical events around the world registration for World Hospice and Palliative Care Day and Voices for Hospices is now open.

Media Workshop

  Palliative Care Training Programme at Rajamundry
     

IAPC will be organising a Media Relationship workshop along with the central council Meeting in Bangalore  on 22nd Augu2009. This is not being planned as the usual ‘media sensitising’ programme. On the other hand it will be the persons from the media who will be addressing the delegates. The faculty will include Mr. Sashikumar ,the chairman of Asian College if Journalism at Chennai, Ms, Ammu Joseph, Independent journalist , Mr. KPM Basheer , special correspondent, The Hindu, Ms. M Suchithra , editorm the Quest Features and Footages and Mr. Mahesh Guptan , Reporter , Malayala Manorama.

We hope that this workshop will help us exploring the role of media in socially relevant issues and how such “media activism” can be used for furthering the palliative care activities in India. This workshop is supported by the Global Advocacy Grant from the Help the Hospices,UK.

 

At G. S. L. Medical College, Rajahmundry, Andrapradesh, a three day workshop in palliative care was held from 18 – 20 June 2009.  
The Programme was jointly organised by GSL Medical College and HelpAge India, Rajamundry. Around 20 interns and junior doctors and staff members of the GSLMC and nurses from the HelpAge India

 
project at Rajahmundry attended the programme
The trained doctors will placed in the community along with the Mobile Medical Unit of Helpage India and also in the Community Care Centre for PLHAs being set up in the Medical College.
The team of trainers was from IPM, Kozhikode and MNJIO& RCC, Hyderabad.  The Community Medicine Department of the G. S. L. Medical College which played a key role in organising this programme are planning to make this a regular event so that all the junior doctors coming out of the medical college will have exposure to palliative care.

Award For Campaign in Palliative Care

       

 

Mr. Mahesh Guptan , Reporter from Malayala Manorama received four awards including the Kerala government media award for Development reporting for studying , preparing and publishing a series regarding palliative care movement in Kerala.

The series was published in Malayala Manorama , a prominent regional language news paper in the country, from April 28 for four days continuously. It was part of a planned activity from the news paper which followed up these report with a month long campaign and helped improve the awareness among the public. During this campaign awareness programmes and volunteer training programmes were held in all 14 districts of the state with the help pf Institute of Palliative Medicine,

 

Calicut and the Kerala Chapter of Indian Association of Palliative Care. More than 5000 volunteers got training through this programme and many of them are now involved in Palliative care activities in different parts of the state.

The other awards received by Mahesh for his work are the following:
The Cardinal Antony Padiyara media award
M. Sivaram award for general reporting
K C Madhava kurup award for general reporting by calicut press club.
The prestigious internal award distributed in Malayala Manorama each year “ the Chief Editor’s Gold Medal”

The Malayala Manorama also launched a website www.vrwithu.com for updated reports regarding Palliative care activities in Kerala.

Observed Annual Trust Day

 
Workshop on Management of Chronic Pain in Children

 

Lakshmi Pain and Palliative Care Clinic, Chennai observed its Annual Trust Day on April 16th 2009. On this day bookmarks highlighting about Palliative Care were distributed to  various bookshops to spread awareness among the public .
The centre co-organised a public awareness program in view of National cancer Survivors Day at Govt. General Hospital along with the dept. of Gastroenterology and Dept. of Health and Family Welfare, Govt. of Tamil Nadu on 6th June 2009. The audience consisted of cancer survivors and supporters of the welfare of such people. The message was 'Celebration of Life' and all present took a pledge to work for the welfare of patients who suffer from cancer.

First State Level Workshop on Pediatric Palliative Care with the theme, “Management of Chronic Pain in Children”, was conducted at the Institute of Palliative Medicine, Kozhikode on April 26th 2009.

The workshop was organised jointly by C4CCCI (Caring for Children with Cancer and Chronic Illness), Department of Pediatrics, Medical College, Calicut and Indian Academy of Pediatrics(IAP), Kozhikode Branch.
More than 70 pediatricians attended the day long workshop which was inaugurated by Dr. V.K. Parvathy, President IAP, Kerala

     

World conference on peadiatric Curative, Palliative and Hospice Care

 
Childrens Hospice International’s 20th World Congress (CHI World Congress):
14th- 16th September 2009

 The theme of this congress is "Nurturing the Spirit" but the congress intends to fully explore all aspects of children's palliative care from the latest innovations in clinical care and new research to theories and experiences with such issues as "Pain of the soul" and "Caring for the Caregivers".
